Medicare and Chronic Care Management
At Jupiter Internal Medicine Associates, one of the many benefits offered by Medicare is Chronic Care Management.  Health care providers may be  paid to manage two or more chronic medical conditions that meet Medicare criteria.  This additional medical support is a proactive approach to aging in place.  The  goal to keep patients out of the hospitals is keeping patients active and improving their quality of life who live with chronic illness.  A comprehensive care plan and a signed agreement is required by our health care providers to review with each patient that is interested in participating in this program.
Cost for Chronic Care Management
The cost to participate in chronic care management may be a monthly fee depending upon the Part B deductible and copay insurance applicable. Â Patients that have a supplemental insurance may help to cover and off set the monthly fee.
